Heat olive oil in a large saucepan and sauté onions and carrots until translucent.

Stir in tomatoes and garlic; simmer uncovered for 30-45 minutes until the sauce reduces, breaking up whole tomatoes as needed.

Concurrently, slice and brown Italian sausage in a separate skillet, then drain and set aside.

Remove the sauce from heat and purée until smooth using a food mill or processor.

Return the puréed sauce to the pot, add browned sausage, and simmer for 20 minutes.

Season with salt and sugar to taste, then toss with hot cooked pasta and serve immediately.
