Brown ground beef and onions in a cold skillet over medium heat, breaking the meat into small pieces.

Add garlic and green pepper; cook, stirring, for 2 minutes.

Stir in 1 cup of water, bring to a simmer, and scrape the pan to loosen any browned bits.

Combine ketchup, brown sugar, mustard, Worcestershire, salt, pepper, and the second cup of water.

Bring to a simmer, then reduce heat to low and cook for 30-45 minutes until most liquid evaporates and the mixture thickens; adjust seasoning as needed.
